UnifiedCare Community Health Center Health sliding fee discount

 UnifiedCare Community Health Center (UCCHC) is a Safety-net provider to the
underserved low-income population and provides outpatient primary care services to
all patients regardless of ability to pay. The Sliding Fee Discount Program provides a
reduced fee schedule for basic comprehensive primary and preventative medical,
behavioral health, and dental services based on the patient’s ability to pay. This
sliding fee scale program is available to all patients who qualify, based on family size
and income. Federal and State regulations require the use of a Sliding Fee Schedule
based on poverty guidelines as published annually by the Federal Register. All
patients whose income, based on family size, is at or below 200% of the federal
poverty level will be offered services based on our sliding fee schedule. The nominal
fee for patients who qualify for UCCHC’s Sliding Fee Discount Program, with
incomes of 100% or less of the Federal Poverty Guidelines, is $30.
